Minnesota OCM Portal Registration Guide
After your doctor certifies you, you must complete registration with the Minnesota Office of Cannabis Management. This guide walks you through every step of the state portal.
Important: Doctor ≠ Your Card
Your doctor only certifies that you qualify. You must then complete state registration (this guide) to receive your actual medical card. Many patients don't realize this extra step is required.
No State Fee Since July 2023
Minnesota eliminated the annual patient enrollment fee on July 1, 2023. The state fee is $0. The remaining charge is the practitioner visit. Leafwell's public certificate price is $139, and Hub code MNHUB is configured at $99.
Before You Start Registration
This form is the state application, not the Leafwell or PrestoDoctor visit. The full roadmap is on the medical card guide. If the ID address is missing or stale, Leafwell's page lists residence documents the form has asked for, including a current lease, a recent utility bill, prior-year state tax documents, or a notarized affidavit. Confirm the list on the form.

- Valid photo ID (driver's license, state ID, or passport)
- Proof of Minnesota residency (if ID is from another state)
- About 15 minutes of uninterrupted time
Timeline
- Practitioner visit: The provider's own schedule. Leafwell says under 10 minutes. That is Leafwell's claim.
- Enrollment email: Use the link and check spam. Leafwell says within 3 business days.
- Submit the form: While the certification is less than 90 days old. Apply within 60 days.
- Office decision: Up to 30 days after OCM receives the application and certification.

A submission receipt is not approval. Minn. Stat. 342.52 says the office must approve or deny enrollment within 30 days after it receives the application and certification. The Office of Cannabis Management says review of a completed application may take up to 30 days. Enrolled patients use the registry verification in their account. Minnesota does not mail a plastic card.
Tip: Leafwell says the first enrollment email can arrive within 3 business days. That email starts the application. It is not the approval email.
Warning: Do not go to a medical dispensary on the strength of a submission receipt.

After you are enrolled, complete the patient self-evaluation in the registry before a medical cannabis dispensary visit. Bring a government photo ID. A caregiver who picks up medicine brings their own ID. Leafwell tells patients to meet a dispensary pharmacist and to bring a medication list plus the certifying visit summary to the first appointment.
Tip: Health insurance does not pay for medical cannabis.
Common Mistakes to Avoid
These are the most common issues r/MNtrees users report. Don't make these mistakes!
Treating the submission email as a card
Reality: Minnesota does not mail a plastic card. You shop after the office approves enrollment and you can open the registry verification. A receipt that the form was submitted is not that approval.
Thinking the doctor gave you the card
Reality: The doctor only certifies you. You must complete state registration separately.
Searching for a public signup form
Reality: New patients should use the enrollment link in the email. People who already have an account use the registry login. Those are different doors.
Paying a "state fee" to a website
Reality: Minnesota eliminated the state fee on July 1, 2023. If any site asks for a state fee, it's a SCAM.
Using a different name than on your ID
Reality: Your name must match exactly between doctor certification, state portal, and ID.

- You'll receive a confirmation email (usually within 24 hours)
- This email IS your medical card - save it to your phone
- You can visit any Minnesota dispensary immediately
- Bring photo ID + email confirmation to the dispensary

Screenshot or save your confirmation email to your phone's photo gallery for easy access at dispensaries. Some patients also print a copy for their wallet.
Most dispensaries have seen the email format before and will accept it without issue.
Troubleshooting Common Issues
"I never received the email from OCM"
- Check spam/junk folder (most common issue)
- Wait 48 hours from your doctor visit
- Contact your certifying provider to confirm submission
- Call OCM Patient Services if still no email after 48 hours
"My registration link expired"
Contact OCM Patient Services to request a new registration link. Have your name, date of birth, and certifying provider information ready.
"My photo upload keeps failing"
- Ensure the image is clear and well-lit
- All four corners of the ID must be visible
- Try a different browser (Chrome works best)
- Reduce file size if image is too large (under 10MB)
